If you use the contact form, the site receives the name, optional reply email, subject, message, and the page you contacted us from. The server also records the request user-agent with the message so the team can operate and protect the contact channel.
Public browsing does not require an account. Optional email-link sign-in is used for protected site functions. Authentication and account data are handled through the site's authentication provider.
Google Tag Manager is loaded only when the site is configured with a Tag Manager ID. The tags and tools enabled through that configuration determine what analytics or marketing requests are made. Clinic websites, maps, video, messaging links, and other external services have their own privacy practices.
Do not use the contact form to send emergency information, medical records, payment details, or anything you would not want stored as a support message. For corrections or questions, send only the minimum detail needed for the team to understand the issue.
